Body
Background: Coronary heart disease is the leading cause of death in Kansas. Sedgwick County has higher rates of diabetes and heart disease than the state of Kansas. To address these adverse health outcomes, the Health Department seeks to strengthen its partnership with community agencies and KDHE to support implementation of a coordinated approach to prevent and control diabetes, heart disease, obesity, and associated risk factors.

Sedgwick County will be one of seven communities consisting of eleven (11) counties (Allen, Crawford, Finney, Jewell, Johnson, Lincoln, Mitchell, Republic, Sedgwick, Smith, and Wyandotte) selected by KDHE to implement 15 collaborative strategies aimed at reducing these chronic diseases and associated risk factors. This project will utilize environmental approaches to promote health, reinforce healthful behaviors, implement health system interventions and community-clinical linkages; these approaches will lead to sustainable efforts within Sedgwick County.

The project is intended to span four years with initial funding beginning December 2014. Funding for the consecutive three years is contingent on continued availability of funds.
If approved, a portion of this funding, approximately $204,306 or 35%, will be used to add 3.0 FTEs. These positions will be used to implement the project, coordinate community collaboration and analyze chronic disease data on the local level. The remaining funds will be used to s...
