Body
Background:  Coronary heart disease is the leading cause of death in Kansas. Sedgwick County has higher rates of diabetes and heart disease than the state of Kansas. To address these adverse health outcomes, the Health Department seeks to strengthen its partnership with community agencies and KDHE to support implementation of a coordinated approach to prevent and control diabetes, heart disease, obesity, and associated risk factors.
Sedgwick County will be one of seven communities consisting of eleven (11) counties (Allen, Crawford, Finney, Jewell, Johnson, Lincoln, Mitchell, Republic, Sedgwick, Smith, and Wyandotte) selected by KDHE to implement 15 collaborative strategies aimed at reducing these chronic diseases and associated risk factors. This project will utilize environmental approaches to promote health, reinforce healthful behaviors, implement health system interventions and community-clinical linkages; these approaches will lead to sustainable efforts within Sedgwick County.
 
The project is intended to span four years with initial funding beginning December 2014.  Funding for the consecutive three years is contingent on continued availability of funds.  
If approved, a portion of this funding, approximately  $204,306 or 35%, will be used to add 3.0 FTEs.  These positions will be used to implement the project, coordinate community collaboration and analyze chronic disease data on the local level. The remaining funds will be used to support subcontracts with community organizations to assist with implementation of project strategies. The Health Department does not have capacity to carry out these duties without additional staff.
 
Alternatives:  Not to enter into the agreement and accept the funds and responsibility for the coordinating agency for the project.
 
Financial Considerations:
The agreement shall become effective immediately, once all parties have executed said agreement.  The agreement shall conclude on September 30, 2015.  The initial payment of $150,000 will be distributed upon receipt of signed agreement.  A maximum of up to an additional $430,000 in funding availability; payments distributed quarterly based on timely submissions of affidavits of expenditures.  
 
Cost Center:  Funds to be managed through a new established fund center and internal order.  
 
Budget Authority Adjustment:  Additional budget authority in the amount of $580,000 is being requested per the attached budget form.  
 
Personnel Consideration:  The request is to add 3.0 FTEs to the Health Department staffing table.  Total cost for this staffing table adjustment is estimated to be $204,306. See Attachment 1 - Table 1.  Budget impact statement is attached.
Legal Considerations:  The County can enter into the grant agreement pursuant to its home rule powers under K.S.A. 19-101 et seq.  Pursuant to the County's Grant Applications and Awards Policy, this grant agreement requires approval of the Board of County Commissioners.  Approval is by simple majority vote.
