Body
Recommended Action: Approve the contracts and authorize the Chairman to sign.

The Central Plains Area Agency on Aging (CPAAA) will enter into a provider agreement with Butler County Department on Aging (BCDoA) to provide case management services to Butler County residents for the Senior Care Act (SCA) program. The agreement details how the agency will provide case management services to Butler County residents 60 years of age or older who have functional limitations which restrict their ability to carry out activities of daily living and impede their ability to live independently. The SCA program was established by the Kansas Legislature to assist older Kansans to live independently within the community with support services. The SCA program provides assessment, case management, and coordination of in-home services to eligible participants.

Alternatives: Not approving this agreement would require CPAAA to provide case management services to SCA participants in Butler County.

Financial Considerations:
Contract period starts: July 1, 2018
Contract period ends: June 30, 2019

The program is funded through the Kansas Department on Aging and Disability Services (KDADS). The Contractor agrees to accept compensation at the rate of $25.00 per unit for case management services as established by Central Plains Area Agency on Aging (CPAAA) for the Senior Care Act Program. A billable unit is fifteen (15) minutes of case management services.
