Service Agreement with the Association of Community Mental Health Centers of Kansas (ACMHCK).

Recommended Action: Approve the agreement and authorize the Chairman to sign.
Background: Care Management Technologies (CMT) utilizes actual Medicaid encounter data. This data assists COMCARE in identifying patients who are at highest risk for hospitalization, provides information regarding patients’ compliance with picking up prescriptions from pharmacy, number of hospitalizations, cost of care, and similarities in prescriptions across multiple providers.
This data is collected over time, allowing COMCARE to focus resources where they are most needed and provide interventions to those patients at the highest risk. This data also provides information on patient’s use of emergency rooms and inpatient units. This data will allow COMCARE to provide interventions that will help patients find alternative resources, decreasing the use of higher levels of care. This system is a risk stratification software tool that helps COMCARE provides better and timelier care.
Alternatives: If this agreement is not signed COMCARE will not have access to this data and lose an opportunity to focus services where they are most needed.
Financial Considerations:
Term: 12-1-17 to 10-30-18
Cost: $12,850 initial year, additional year’s access fee will be $21,850.
Fund Center: 31101-252
Legal Considerations: The authority for this action is K.S.A. 19-101 and a simple majority vote is required.
Policy Considerations: All Sedgwick County policies have been followed in the development of this agreement.
