Recommended Action: Approve agreement and authorize the Chairman to sign the contract.

Background: In 2014, a total of 1,950 patients were diagnosed with depression or anxiety by primary care providers, followed by 1,661 in 2015. In addition, there were 481 patients in 2014 and 715 patients in 2015 that were diagnosed with alcohol and/or other substance abuse disorders. In response to this information, GraceMed requested funding from the Health Resources and Services Administration (HRSA) to improve and expand the delivery of comprehensive, culturally competent, collaborative, and integrated Substance Use Disorder (SUD) services with a focus on Medication-Assisted Treatment (MAT) for underserved adults who reside in our service area.

This project will be called Pathway to Recovery and GraceMed has asked COMCARE to partner in this project. COMCARE will provide patients a combination of MAT and behavioral therapies. Together, GraceMed and COMCARE will provide an evidenced-based, recovery-oriented system of care to give patients the resources, guidance, and on-going treatment needed to rebuild their lives, health, and relationships.

This agreement will provide funding for COMCARE to hire a Behavioral Health Consultant (BHC) and a Case Coordinator to provide services as outlined in the partnership agreement.

Alternatives: If this agreement is not approved COMCARE will not have the ability to partner with GraceMed and GraceMed will have to seek another partner for this program.

Financial Considerations:
Funding: $268,250 per year (two-year contract)
Fund Center: new cost center
Term: 7-1-16 to 6-30-18

Budget Authority Adjustment: Additional budget authority of $20,170 is being requested for the 2016 budget and $80,676 for 2017. A new fund center will be created.
