Affiliation Agreement with Phoenix Home Care & Hospice.

Recommended Action: Approve the agreement and authorize the Chairman to sign.
This is a standard agreement that serves to affiliate providers of Intellectual and Developmental Disability (I/DD) services in Sedgwick County with the Sedgwick County Developmental Disability Organization (SCDDO). Affiliated organizations are licensed or contracted through the Kansas Department for Aging and Disability Services (KDADS) and have met SCDDO standards. The SCDDO contracts with KDADS to function as the official Community Developmental Disability Organization (CDDO) for Sedgwick County and is responsible to ensure quality services are available for eligible individuals with intellectual and/or developmental disabilities.
This developmental disability service agreement is with Phoenix Home Care & Hospice. The agreement was necessitated by an individual moving to Sedgwick County and requesting to maintain their current provider. This agreement will allow the individual to maintain their provider without disruption of services. Typically, all potential affiliates must first be approved by the Intellectual and Developmental Disability Advisory Board (IDDAB) but to ensure no interruption in services it was not possible for IDDAB to review and approve this applicant prior to contracting; therefore, this contract is formally contingent on approval of the affiliate by the IDDAB at their May meeting.
Alternatives: The SCDDO is required to contract with qualified service providers that have been selected by an individual who is eligible to receive intellectual and developmental disability services. This provider has been selected as the preferred provider for one individual but will be open to additional referrals as requested. Without this agreement the individual would be required to select a new provider.
