CONSIDERATION OF  A SUNFLOWER FOUNDATION GRANT IN THE AMOUNT OF $200,000.00 FOR GRACEMED AT COMCARE INTEGRATED CARE IMPLEMENTATION.

Grant Summary and Timeline: COMCARE and GraceMed will be implementing a reverse co-location integrated care model by expanding GraceMed's scope of work as a Federally Qualified Health Clinic by placing a new GraceMed clinic within COMCARE's Outpatient Service located at 1919 N. Amidon. This location is ideal because it is COMCARE's largest service location by patient volume and even more specifically as it is where a  large number of medical service providers reside and will therefore naturally increase collaboration between medical teams. Providers will work collaboratively and share ownership of patient outcomes. Collaborative consultations will be routine practice and will have consistent screening of both behavioral and physical health needs. COMCARE and GraceMed have agreed to use their resources such as utilizing both of their Health Home Staff to provide necessary care management/coordination and health promotion activities for chronic conditions as warranted. GraceMed will also utilize their existing referral specialist to access specialty and other care in the community.

 

The $200,000 grant is a two year grant that will help fund one Mental Health Clinician and a portion of one Medical APRN. COMCARE will utilize the grant to fund  a Mental Health Clinician and allocate the remaining funds to GraceMed for the funding of a Medical APRN. Through the process of planning for the implementation COMCARE and GraceMed have become committed to the development of an integrated care clinic that will be more than a co-location and closer to a system level integration.

Budget Authority Adjustment: Additional budget authority is being requested for the 2016 budget in the amount of $43,979.

 

Personnel considerations: There will be no changes to COMCARE's staffing table. COMCARE will utilize a currently vacant position to fill the Mental Health Clinician position.

 

Expected measurable outcomes:

Improved health status

Increase Behavioral Health patients accessing primary care.

 

At this time we have determined that at minimum we will be capturing the following on all patients:

•      Patient Health Questionnaire 2 (PHQ-2) for depression screening

•      Patient Health Questionnaire 9 (PHQ-9) for depression screening if positive response to PHQ-2

•      Generalized Anxiety Disorder 7 (GAD-7) for anxiety screening

•      SBIRT for substance use screening

 

 

Alternatives: If this grant application is not approved funding for a Mental Health Clinician and Medical APRN will not be available. COMCARE and GraceMed will have to find another source of funding for these positions.

 

Policy considerations: All Sedgwick County policies have been followed in the development and review of this agreement.

 

Legal considerations: The authority for this action is K.S.A. 19-101 and Resolution 64-2013.
