Title
Four vendor contracts to provide home care and TeleHealth services for the Sedgwick County In-Home Program (SCIP).
Body

Recommended Action:  Approve the contracts and authorize the Chairman to sign.

Background:  The four contracts are for the Sedgwick County In-Home Program (SCIP).  SCIP was established by the County to assist residents age 60 and older with functional limitations which restrict their ability to carry out activities of daily living and impede their ability to live independently.  Consumers share in the costs of services based on their income. Co-pay amount is determined  based on a sliding fee scale.  The SCIP program assists residents in need of attendant care, homemaker and TeleHealth services allowing them to remain in the community and in the home environment of their choice.

The contracts are a renewal with a contract period beginning January 1, 2015 through December 31, 2015 with the following providers:
1.      All Saint's Home Care, Inc.
2.      Right At Home
3.      St. Raphael Nursing Services, Inc.
4.      Windsor Place

Financial Considerations:  The total budget for SCIP is $155,229.  The program is funded through Sedgwick County Aging Mill Levy funds budgeted and approved in the 2015 adopted budget by the Commission on 8/13/2014.

Funding Source:
      Cost Center:  34022-205 In-Home Services
