Title
Agreement renewal with United Methodist Open Door (UMOD) to provide Case Management Services.
Body
Recommended Action: Approve the Agreement, and authorize the Chairman to sign.
Background: Since late 2007, United Methodist Open Door (UMOD) has successfully applied for and received a grant to provide a permanent supportive housing project through the US Department of Housing and Urban Development (HUD). This grant has again been renewed. The goal of the project is to support and assist homeless individuals so that they can transition out of homelessness and into a more stable environment. One component of the grant is a dedicated mental health case manager to work with approximately ten to twelve people in the UMOD housing project who have a severe and persistent mental illness. To that end, COMCARE provides .65 FTE to serve these consumers as a subcontractor for case management services. The HUD grant funds $15,818.00 for the .65 FTE for one year, and this position is currently filled. As a case manager, the position has the ability to produce fee-for-service revenue which helps to address the costs not covered by HUD.
This agreement is a renewal. The only change to this year's contract was to remove outdated HUD language regarding leverage amounts, which was no longer applicable. Grant and match totals remain the same.
Alternatives: The HUD grant represents an opportunity to support an important housing resource in Sedgwick County. Without COMCARE's participation, UMOD might not be able to secure the same level of high quality case management services.
Financial Considerations:
Revenue: $15,818.00
Cost: (Match requirement) $3,955.00
Funding Source: 31071-252
Term: 04/01/2012- 03/31/2013
Legal Considerations: The authority for this action is K.S.A. 19-101 and a simple majority is required.
Policy Considerations: All Sedgwick County policies have been followed in the development of this renewal agreement.
