Central Plains Area Agency on Aging (CPAAA) has operated the Client Assessment Referral and Evaluation (CARE) program since January 1, 1995. Under this program, CPAAA is responsible for coordinating timely and accurate completion of all nursing home pre-assessments in Sedgwick, Harvey, and Butler counties in accordance with the KDADS agreement and State and Federal laws. Customers and their caregivers are given information about in-home services at the time of the assessment and some choose to receive services in the home. This program currently maintains a diversion rate of 24.3 %, which is higher than the state average of 22.9%. The agreement establishes the rate that the CPAAA is reimbursed for each assessment, which covers the program expenses for three full time employees.

Alternatives: The Commission could decide not to approve the agreement and the CARE program activities would be delayed.

Financial Considerations: This program is fully funded through reimbursement from the state at a unit cost of $186.56 for each contracted community based assessment and $88.53 for each hospital based assessment. There are no financial obligations for the County. Cost Center is CARE 34004-254 (Internal Order Number 340046-13).

Legal Considerations: The Service Provider Agreement for the CARE program is authorized by K.S.A. 39-968 and may be approved by simple majority vote.

Policy Considerations: CARE is a mandatory state program. This contract allows CPAAA to effectively manage a program funded by KDADS in Sedgwick, Harvey and Butler counties.
