REQUEST TO RE-INSTATE FUNDING FOR COMMODITIES IN THE AMOUNT OF $138,143 TO THE SEDGWICK COUNTY DIVISION OF HEALTH ANNUAL BUDGET FOR 2017 TO PROVIDE TRAVEL IMMUNIZATIONS.

RECOMMENDED ACTION: Approve the request to reinstate funding in the amount of $138,143 to purchase travel immunizations and authorize the Chair to establish budget authority.

Background:
The Division of Health (DOH) has provided Travel Immunizations to residents of Sedgwick County of all ages since before 2002. Travel Immunizations include Rabies pre-exposure, Typhoid, Yellow Fever, and Travel Immune Serum Globulin (TISG) vaccines. Travel Immunizations are not provided by Community Health Clinics (CHC) but are provided by pharmacies and physicians. However, not all travel shots are provided by all retail storefront medical providers in Sedgwick County.
In 2015, 1,281 travel shots were provided by DOH staff. Total actual expenditures for 2015 totaled $159,551, of which $138,143 was expended on commodities for the immunizations themselves. Total actual revenue for 2015 totaled $137,660, or 86.2 percent of total expenditures; resulting in a net expense of $21,891 to the County General Fund. Revenue comes from patient fees because many insurance companies do not cover travel immunizations. Fees for all vaccinations, including travel vaccines, were increased as of 10/1/2016 to cover cost of vaccines plus 15%.
The number of immunizations and encounters show that it takes approximately 0.43 FTE (0.21 FTE Fiscal Associate and 0.22 Registered Nurse) to maintain travel shots. However, both of these positions have other duties in addition to providing travel immunizations and each position’s duties cannot be provided by the other. It was determined therefore not to cut staffing when travel vaccines were eliminated from the budget. These positions continue to provide services. No additional staffing is needed.
Travel vaccine stock on hand as of 1/3/17 is limited to 1 Rabies Pre/Post Exposure vaccine ($225.76).
Alternatives:
To not approve the request for $138,143 to provide travel immunizations be re-instated to the Division of Health adopted budget for 2017.
Financial Considerations:
General Fund funding in the amount of $138,143 to be re-instated to the 2017 budget for the Division of Health. Request for a new fund center to be established for tracking expenditures and revenues for Travel Immunizations exclusively.
Budget Authority Adjustment: Additional budget authority in the amount of $138,143 is being requested for the 2017 budget to be transferred from the Operating Reserve.
Legal Considerations: Approval is by simple majority vote.
