Grant Summary and Timeline:

The Chronic Disease Risk Reduction (CDRR) program within the Kansas Department of Health and Environment (KDHE) announced additional supplemental funding is available for selected local health departments to increase opportunities for physical activity to support healthy community behaviors and build community support to increase chronic disease self-management education within Kansas counties.   This funding is restricted to local health departments or their designees currently funded through the 2014-15 State Fiscal Year (SFY).

This funding will allow the Health Department to implement and pilot the Earn-A-Bike program aimed at increasing physical activity and increase efforts to promote biking in the community. In addition, this funding will allow continued educational opportunities for the chronic disease self-management program and provide support for people living with these diseases within the county.  Sedgwick County has a higher rate of chronic diseases, particularly in the area of diabetes and diseases of the heart as compared to the state according to the 2012 Behavioral Risk Factor Surveillance Survey (BRFSS Data). (See table below)

Type of Chronic Disease            Sedgwick County      State of Kansas

Diseases of the Heart                              5.5%                              4.5%

Diabetes                                                    10%                               9.5%

This supplemental grant request offers funding and technical assistance to communities that seek to implement best practices and evidence-based models for increasing opportunities for physical activity and expanding chronic disease self-management education programs.
