CONSIDERATION OF A GRANT IN THE AMOUNT OF $500,000 TO THE HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA), ON BEHALF OF GRACEMED HEALTH CLINIC, TO BUILD A NEW SCHOOL-BASED HEALTH CENTER AT JARDINE MIDDLE SCHOOL, LOCATED IN WICHITA, KANSAS, FOR THE HEALTH DEPARTMENT.

Grant Summary and Timeline: The Health Department has partnered with GraceMed Health Clinic, a Federally Qualified Health Center (FQHC), to ensure the provision of quality health care for underserved children in Wichita since 1998. In 2010, the Health Department agreed to act as a sponsoring organization for GraceMed to receive a two-year HRSA School-based Health Care Capital (SBHCC) grant to partially fund the architectural, site preparation and construction costs to build a new 4,500 square foot school-based health center at Cloud Elementary School. The Cloud Clinic, which replaced the Evergreen Health Clinic located adjacent to Cloud, is currently in design stages. The architect is scheduled to present the final drawings to USD 259 School Board at their next scheduled meeting. Once approved, USD 259 will solicit bids from general contractors to begin construction by November 2012, with a planned completion date of June 2013.
GraceMed respectfully requests the Health Department, again, act as the sponsoring organization for a new SBHCC grant from HRSA to build a new school-based clinic at Jardine Middle School, 3550 Ross Parkway, Wichita, KS, 67210. The target population for the new clinic includes 27,047 people living in eight census tracts adjacent to the clinic.
Of these residents:
• 6,377 (or 23.5%) live at or below 100% of Federal Poverty Level (FPL)
• 7,221 (or 26.7%) are children, age birth to 19 years
• 6,268 (or 23.2%) have, or are eligible for HealthWave (SCHIP) insurance coverage
• Another 5,571 (or 86.8%) of the 6,415 children enrolled in Jardine and eleven nearby schools qualify for the federal free/reduced lunch program.
The 4,500 sq ft clinic will have six medical exam rooms and one dental operatory to accommodate two medical providers and a dental hygienist. This new access point will serve as a medical home for low-income children and their families in southeast Wichita; both uninsured and those eligible for HealthWave (SCHIP) state insurance coverage.

Budget Authority Adjustment: Yes, for $508,600. The Health Department will act as a pass-thru for funding; to be managed through the establishment of a new Fund Center. No matching funds are required. No county funds are required. Sedgwick County will receive a 1.75% administration fee of $8,600 for managing and distribution of funding. The grant is for the period of December 1, 2012 through November 30, 2014.
Sedgwick County and GraceMed Health Clinic will enter into a Memorandum of Understanding (MOU) Agreement specifying partner responsibilities and payment processing guidelines. GraceMed, in partnership with USD 259, will have oversight of the project. Land for the project is owned by USD 259. Total building costs are estimated at $967K, leaving a balance of $467K due after the grant. GraceMed will be responsible for securing and providing all funding (i.e. raising funds from the community, use of their own cash reserves, or secure financing by negotiating a low-interest rate loan through USD 259) to complete the project.

Analysis:
School-based health centers are a major component of the nation's health care safety net, enabling children with acute/chronic illnesses to attend school while also improving the overall health and wellness of all eligible children and their families through health screenings, health promotion and disease prevention activities. School-based clinics serve the students and families from the host school as well as surrounding schools.
Services provided at Jardine Middle School will be:
--- Preventative oral health care (e.g. cleanings, fluoride varnish, sealants)
--- Referrals to GraceMeds main dental clinic and/or private dentists for restorative care
--- Physicals (e.g. sports, KanBeHealthy, well-child checks, immunizations)
--- Wellness & prevention (e.g. prenatal care, developmental screenings [including behavioral health], pap smears, breast exams, prostate checks)
--- Disease and chronic disease management (diabetes, heart disease, asthma, high blood pressure, depression, sexually transmitted disease)
--- Urgent & acute care (e.g. ENT infections, colds, flu, childhood diseases, sprains)
The impact on our community includes:
--- Providing health care access to 3,100 new patients through an estimated 9,400 patient visits
--- Permanent positions for at least eight (8) FTE new health care workers
--- Construction jobs for 8.3 FTE through the duration of the project
--- $967,000 will be invested in the local economy through building construction and equipment purchases.
Expected measurable outcomes:
The goals of the SBHCC project are in line with the goals of the SCHD:
• Increase the number of uninsured residents with an established primary care medical home
• Provide more capacity and additional health care access points for low-income children, both uninsured and those eligible for HealthWave (SCHIP) insurance coverage
• Provide strategically located primary care clinics with extended hours as an alternative to local emergency departments for non-emergency care
• Establish new/expanded access points for newly uninsured residents to receive care.
Alternatives: To not apply for the funding, decreasing the chances of new funding coming into Sedgwick County to expand needed school-based health services.

Legal considerations: Authority to enter into the Memorandum of Understanding with GraceMed Health Clinic for the grant funds is established by K.S.A. 19 -101. Please see attachment No. 5, Funding Opportunity Announcement by H.H.S. issued May 9, 2012 for federal authority for the SBHCC program. Approval is by simple majority vote.
