Grant Summary and Timeline:  Control of TB is a core public health function provided by the Health Department and mandated by State statute (see Legal Considerations section below). Employers, universities, detention centers, hospitals, nursing homes, homeless shelters and physicians refer all persons with positive TB test results, and those suspected of active TB, to the Health Department.  Persons exposed to known active TB cases are sought out and evaluated.

This award provides funding necessary:

• To assist with the management and treatment of people with active TB through direct observation of daily therapy;

• To evaluate and treat contacts for TB Infection and suspects for active TB disease; and

• To provide preventive therapy for those with TB Infection but not transmitting TB to others; and

• For staff to attend TB training, which consists of five team members and contracted physician services to educate, evaluate, treat and monitor TB, and provide community services.

The TB clinic averages 240 encounters monthly; and as of December 24, 2014, the TB clinic has:

• Evaluated 371 clients

• Tracked 40-50 non-client suspects

• Managed 15 cases of active disease

• Provided direct observation therapy 1,504 times

• Placed 101 clients on preventive therapy

Budget Authority Adjustment:  No adjustment requested. Sufficient budget authority exists within 38013-274, KDHE TB Control, for 2015.

Personnel considerations:  This grant funding is used to fund a portion of salaries for one full time community health nurse and one full time medical assistant.

Expected measurable outcomes:

By 12/31/15, we expect to maintain the following outcomes for all reported and evaluated active TB cases in Sedgwick County:

     • 95% of cases will start or complete therapy within the period specified by the physician managing their care

     • 90% of contact investigations will be completed within 6 months of case report date

     • 85% of all high-risk Latent TB Infection cases that consent to begin treatment complete therapy within one year

Alternatives:  To not accept the grant funding and increase the County budget accordingly or to reduce the program staffing and training commensurate with the loss of this grant award, which would limit the ability of the Health Department to control the spread of TB in Sedgwick County.

Policy considerations:  Compliance with the statutory mandates established in Kansas State Statutes.

Legal considerations:  Legal authority is established by the following Kansas Statutes: K.S.A. 65-116a et seq, -119, -129a et seq, and -202. The grant award agreement may be entered into pursuant the County's home rule authority within K.S.A. 19-101 et seq.  Furthermore, pursuant to the County's Grant Applications and Awards Policy, due to the dollar amount of this grant and this being a grant renewal, the County Manager is authorized to approve the grant.
