Title
Addendum to Developmental Disability Services Agreement.
Body
Recommended Action: Approve the addendums (6) and authorize the Chairman to sign.
Beginning July 1, 2014, the State of Kansas initiated a health home program for individuals on Medicaid who have a serious mental illness (SMI). Health Homes provide comprehensive and intensive coordination of care to improve outcomes for individuals with challenging health issues. Individuals with intellectual and/or developmental disabilities (I/DD) who are participating in the SMI Health Home can maintain their Targeted Case Manager (TCM) but only if the designated Health Home Partner contracts with the TCM agency. The Sedgwick County Developmental Disabilities Organization (SCDDO) has worked with the local TCM agencies to ensure they are ready to provide services under the health home model of care.
COMCARE is offering health home services through their program Health Links. Health Links will need to contract with TCM agencies to ensure individuals with I/DD have access to their TCM and that TCM agencies receive payment for the services. SCDDO currently contracts with all the local TCM agencies and has worked with COMCARE to amend the current agreements to include provision and payment for services by TCM agencies to individuals with I/DD assigned to the Health Links health home (addendum).
Agencies participating in the Health Links health home through this addendum include: Cerebral Palsy Research Foundation of Kansas, Goodwill Industries of Kansas, Medicalodges - DBA New Hope, Rainbows United, Starkey and The Arc of Sedgwick County.
Alternatives: The addendum outlines the requirements for provision of services by TCM agencies for individuals with I/DD assigned to the Health Links health home. Without this addendum, COMCARE would have to initiate contracts with all TCM agencies independently. Additionally, TCM agencies would not be paid for services to individuals in the Health Links health home until these contracts were negotiated and approved.
Financial Considerations: Payment to TCM agencies is processed through COMCARE and made only after receipt of payment from Medicaid.
Rate: $137.32 per member per month for individuals with I/DD assigned to Health Links when at least one service has been provided and documentation completed.
Cost Center: 310996-15
Legal Considerations: The authority for this action is K.S.A. 19-101 and a simple majority
vote is required.
Policy Considerations: All Sedgwick County policies have been followed in the development of this addendum.
