Title
AMENDMENT TO THE CAPITAL IMPROVEMENT PROGRAM (CIP) FOR "COMCARE PRIMARY INTEGRATION PROJECT."
Presented by: Tania Cole, Program Manager, Project Services.
RECOMMENDED ACTION: Approve the amendment to the 2014 Capital Improvement Program.
Body
COMCARE is submitting a CIP amendment for the Primary Care Integration project. Some background on this initiative is to provide necessary means for primary care providers from COMCARE's community partner, GraceMed, to be co-located within COMCARE's community mental health center at a site that clients are already familiar with. COMCARE mental health providers will also be operating from this location. This partnership gives medical and mental health providers the opportunity to collaborate on the treatment, consultation and referral of mutual clients, in turn providing consumers with increased coverage and reducing the treatment gap that typically exists within this population. Similar programs nationally have saved states money, reduced hospitalizations, reduced emergency department usage and produced better overall health outcomes, while increasing individuals' knowledge about their health status.
COMCARE is partnering with GraceMed, a Federally Qualified Health Center (FQHC), who will be co-locating primary care providers to COMCARE's Outpatient Services program location at 1919 N. Amidon which is a leased location. The landlord is aware and has approved of the proposed changes, but will not be contributing financially. GraceMed will pay for their staff that is co-located at this location and bill for the medical services that they provide. COMCARE psychiatric medical service providers will be available for consultation and collaboration on shared patients. COMCARE will bill for behavioral health services provided as a result of this program.
On July 16, 2014, the BoCC formally approved the transfer of budget authority within COMCARE's existing budget to the Primary Care Integration project. The department has set aside $175,000.00 within this budget to cover all associated capital improvement costs. This CIP project will include design and construction for a new restroom, 6 exam rooms, along with flooring, paint, cabinetry, and plumbing which will renovate the leased space (4,981 SF) to provide a medical office environment.
Alternatives: Not approve the amendment and the project would not proceed.
