Grant Summary and Timeline:  Control of tuberculosis (TB) is a core public health function provided by the Health Department and mandated by state statutes. Sedgwick County reports on average nine active cases of TB per year. In 2016, as of June 1, Sedgwick County reports two new active TB cases; in 2015, a total of 13 cases were reported, representing 36% of the cases in the State.

Sedgwick County has a diverse population which includes people from countries which have higher rates of TB and so increased risk for disease spread. In the past five years, 69 percent of the active TB cases in Sedgwick County were in people not born in the United States. Of those, 71 percent had lived in the United States five years or more at the time of their active disease diagnosis. The data does not show an increasing trend of non-United States born people with active TB in Sedgwick County.

This annual grant provides funding to assist with the coordination of and access to care for persons among the indigent population with TB who pose a significant public health risk
.

Sufficient budget authority is included in the 2016 budget; no additional budget authority is being requested. This grant is for the period of July 1, 2016 through June 30, 2017. This grant will be managed through fund center #38035-274; grant funding not to exceed $14,000.

KDHE provides two grants to the Health Department for TB control. This grant, the smaller of the two, is used primarily to contract with the University of Kansas School of Medicine for x-ray services to the indigent. It also pays for laboratory services, reimburses the county for vehicle use for directly observed therapy in the field, and includes some funding for operating supplies.

Personnel considerations:  N/A.

Expected measurable outcomes:

1. In 2016, assure that 95% of all people diagnosed with active tuberculosis start and complete therapy within the period specified by their physician. (In 2015, this goal was exceeded at 100%; in 2016, this goal is currently exceeded at 100%).

2. In 2016, assure that contact investigations are completed for 90% of all people diagnosed with active, communicable tuberculosis within 6 months of the case report date. (In 2015, this goal was exceeded at 100%; in 2016, this goal is currently exceeded at 100%).

3. In 2016, assure 85% of people who test positive for tuberculosis but do not have active disease, and who consent to begin treatment, complete therapy within one year. (In 2015, this goal was met at 88%; in 2016, this goal is currently met at 94%).

Requirements from Sedgwick County:

The Health Department is required to submit affidavit reports to KDHE quarterly within 15 days after the end of each quarter (October 15, 2016; January 15, 2017; April 15, 2017; and July 15, 2017). No further action is required from Sedgwick County for this grant.

Alternatives:  To not accept this grant funding, which would limit the capacity of the Health Department to provide TB testing and treatment services to the indigent population.

Policy considerations:  N/A

Legal considerations:  Pursuant to K.S.A. 19-101a et seq., the County may approve this grant application.  Under the County's grant policies and procedures, the County Manager is authorized to sign this grant application.
