Title
Changes to the COMCARE staffing table to support Health Home services.
Body
Recommended Action: Approve the changes to the staffing table as requested.

Background: In order to provide Health Home services, COMCARE is proposing a change to the current staffing table. Seventeen positions have been identified for elimination to help compensate for the costs of adding one full time APRN (Advanced Practice Registered Nurse), 6 full-time range 127 Care Managers, and 7 full-time range 121 Case Coordinator positions.
Over time, the staffing needs at COMCARE have changed.  Payor sources are now limiting the number of hours of case management services that clients can receive.  This has decreased the number of case management employees necessary to meet demand.  This change in business model has left COMCARE with several vacant Case Manager II positions.  These positions have not been filled recently and are not expected to be necessary in the near future
Alternatives: Health Homes is a core component of KanCare, and COMCARE has requested to be a Health Home partner to provide care coordination for individuals with serious mental illness and other chronic conditions.  Under this model of care, it is necessary to have staff that specialize in both behavioral health and physical health.  This requires positions at a higher range than Case Manager IIs.  Without updating the staffing table, COMCARE will not be a successful Health Home.
Financial Considerations (all based on 2014 annual estimated costs as outlined at an individual position level in the attached budget impact form):
Eliminate 17 Range 120 positions = $874,633.00 in savings
Proposed staffing addition costs
1 FTE APRN (range 136) - $92,608.00
6 FTE Care Managers (range 127) - $393,660.00
7 FTE Case Coordinators (range 121) - $372,435.00
Total Expense - $858,703.00
Cost savings within grant funds: $15,930.00
Below are the proposed positions to be eliminated:
1) Position #20004925: Case Manager II (Funding Source 310616-14)
2) Position #20002930: Case Manager II (Funding Source 310246-14)
