Grant Summary and Timeline:  Control of TB is a core public health function provided by the Health Department and mandated by State statute (See, Legal Considerations below). Employers, universities, detention centers, hospitals, nursing homes, homeless programs and physician offices refer all persons with positive TB skin test or blood test results, and those suspected of active TB, to the Sedgwick County Health Department.  Persons exposed to known active TB cases are sought out and evaluated.

Budget Authority Adjustment: Yes, for $46,932.00; funding will be managed through fund center 38013-274, TB - TB Control. This grant funding is used to fund a portion of salaries for 2.0 FTE (Pos. #20002306 and Pos. #20002307) in the TB Program. (See Table for position funding information.)
 
Personnel considerations:
No additional staff will be requested with this grant funding.
 
Analysis:
KDHE provides two grants to the Health Department for TB control and infectious disease work. This grant, the larger of the two, funds a portion of a nurse position and a medical assistant position that are both responsible for:
 
1) management and treatment of people with active TB through direct observation of daily therapy,
 
2) evaluation and treatment of contacts for infection, and
 
3) provision of therapy for those infected but not transmitting TB to others (latent TB) who are at high risk for TB disease
 
The grant also provides funding for TB training for staff. The TB program consists of five team members and contracted physician services to educate, evaluate, treat and monitor TB and provide community services.
 
In 2012,  as of November 30, a total of 255 clients were evaluated for TB and seven had active disease; 102 have been placed on preventive therapy. The TB program provides TB therapy to 60-90 clients monthly and has an average of 235 monthly encounters.
 
Expected measurable outcomes:
1. In 2013, assure that 95% of all active TB cases that are reported and evaluated for treatment in Sedgwick County have started and completed therapy within the period specified by the physician managing their care.  
 
2. In 2013, assure that contact investigations are completed for 90% of all active TB cases reported in Sedgwick County within 6 months of the case report date in which a contact investigation is indicated.  
 
3. In 2013, assure 85% of all high-risk Latent TB Infection cases reported and evaluated for treatment in Sedgwick County that consent to begin treatment, complete therapy within one year.
 
Alternatives:
To not accept the grant funding and increase the County budget accordingly or to reduce the program staffing and training commensurate with the loss of this grant award, which would limit the ability of the Sedgwick County Health Department to control the spread of TB in Sedgwick County.
 
Policy considerations:
Compliance with the statutory mandates established in Kansas State Statutes.
 
Legal considerations: Legal authority is established by the following Kansas Statutes: K.S.A. 65-116a et seq, -119, -129a et seq, and -202. The grant award agreement may be entered into pursuant to K.S.A. 19-101. Approval is by simple majority vote.
