Grant Summary and Timeline:
The Central Plains Area Agency on Aging (CPAAA) began operation in 1973 as an inter-local agreement between Butler, Harvey, and Sedgwick Counties. The Sedgwick County Board of County Commissioners serves as the governing board for CPAAA. CPAAA is available to assist seniors 60 and over, caregiver of any age, and adults with disabilities through life's transitions and with various levels of support. Our mission is to assist seniors, caregivers and individuals with disabilities to achieve improved health and greater independence.
Requested funds will be used to bring two trainers from Dallas, Texas to Wichita, Kansas to train CPAAA staff on system implementation of the evidence-based HomeMeds™ Medication Management System. HomeMeds™ is a medication alert program designed to prevent hospitalization and health problems caused by medication errors. At any given point in time, CPAAA provides in-home services to 240 individuals age 60 and older living in Wichita. As customers transition off the program, CPAAA accepts new clients to various in-home programs. These individuals are primary targets for the HomeMeds™ Medication Management System. These individuals have health concerns, chronic disease, as well as functional challenges and require formal support to remain in their home.
How this program will help: The majority of adverse drug reactions are preventable. This evidence-based program will help CPAAA staff identify clients who might be at risk for medication misuse or abuse. A comprehensive assessment is conducted using HomeMeds™ software to screen medications for potentially harmful problems. The client's medication is entered into a national database where safety alerts are generated. If one of these alerts pops up for a consumer, a consulting pharmacist contacts the client and their doctor and finds a resolution. Outcomes of the HomeMeds™ Medication Management System show that about 50 percent of all consultations do identify potentially harmful drug interactions. With CPAAA's current case load of 240 in-home clients, this translates into 120 people whose lives and health will be enhanced through use of this program.
