Grant Summary and Timeline:

The Kansas Department on Aging (KDOA) has been awarded two related grants from the Centers for Medicare & Medicaid Services. Through the Hospital Discharge Model (HDM) grant, KDOA has partnered with Central Plains Area Agency on Aging (CPAAA), Independent Living Resource Center, local hospitals and community organizations to develop and implement a hospital discharge model that focuses on discharging patients back to their homes with community-based services, reducing the number of default discharges to nursing facilities. Through the Aging & Disability Resource Center Enhancement (ADRC-E) grant, KDOA will expand and enhance the current project.

 

The HDM/ADRC Grant & The ADRC-E grant budget includes funding for the CPAAA case manager whose time is dedicated to grant activities.

 

This funding is to be used to increase access to older adults in need of services to help them stay in their homes and community.

 

KDOA will provide CPAAA up to $69,429 of funds to provide these services. The funds being provided for the HDM/ADRC-E case manager are 100% federal dollars. There is no state cash match requirement for either the HDM or the ADRC-E grant.

 

The contract year is July 1, 2011 to September 30, 2012. Sub Grant: October 1, 2011 to September 15,2012

 

This grant was not processed by the state until late November, due to personnel changes at the Kansas Department on Aging

Legal considerations: Medicare and Medicaid services are authorized by the Social Security Act, 42 U.S.C. 1936, et. al. Authority to enter into the grant agreement is provided by K.S.A. 19-101. Approval is by simple majority vote.
