Grant Summary and Timeline:
Background: COMCARE of Sedgwick County is applying for a two-year grant for Projects for Assistance in Transition from Homelessness (PATH) through the Kansas Department of Social and Rehabilitation Services. COMCARE has received PATH grant funds since the initiation of the Homeless Program, and they support a variety of services intended to help clients transition from living on the streets to maintaining a home of their own, including:
- Street outreach;
- Screening and diagnostic treatment;
- Case management services;
- Referrals to address other needs, including primary health and addiction treatment services; and
- Technical assistance in applying for housing, benefits, employment, etc.as appropriate.
COMCARE's Homeless Program estimates that the two case managers and two peer providers funded through this grant will outreach approximately 950 clients annually, connecting them to the mainstream resources that they need to find and maintain housing.

Budget Authority Adjustment: Funds received from this grant will be managed through existing funds source 310296-12.
Personnel considerations: This PATH grant will fund two full time outreach case managers, and two part time (0.5 FTE each) certified peer support specialists for two years. One of the two peer support specialists is a newly requested position this year, while the other three positions have been funded through this grant previously. Peer support staff use their lived experience to help engage clients in services, and this practice has proven to be effective and is consistent with current service trends in Kansas.
Expected measurable outcomes: Through the PATH grant, staff will be able to provide the services necessary to help connect literally homeless individuals with severe and persistent mental illness to housing, income, medical insurance, primary medical care, and other resources so that they can start the recovery process.
Alternatives: Without these funds COMCARE's Homeless Program would be unable to maintain the current level of outreach and case management services. Services would have to be reduced or an alternative source of funding would need to be identified.
Policy considerations: All Sedgwick County policies have been followed in the development of this grant application.
Legal considerations: The authority for this action is K.S.A. 19-101. A simple majority vote is required.
