Grant Summary and Timeline: The Chronic Disease Risk Reduction (CDRR) program within the Kansas Department of Health and Environment (KDHE) announced additional supplemental funding is available for selected local health departments to implement and pilot a chronic disease self-management program within Kansas counties. This funding is restricted to local health departments or their designees currently funded through the 2013-14 State Fiscal Year (SFY).
This funding will allow the Health Department to implement and pilot a chronic disease self-management program for people living with these diseases within the County. Sedgwick County has a higher rate of chronic diseases, particularly in the area of diabetes and diseases of the heart as compared to the state according to the 2012 Behavioral Risk Factor Surveillance Survey (BRFSS Data). While efforts are being made to modify behaviors that contribute to these diseases, people living with these diseases also need assistance learning to manage them. To increase chronic disease self-management education and improve the overall quality of life for people in the community, the Health Promotion Program will provide a minimum of six Stanford University Chronic Disease Self-Management workshops. The program will be open to people with a range of chronic health problems such as diabetes and hypertension. Funding for this project will extend from April 12 - June 30, 2014.
The Stanford model is a six week program offered in community settings such as senior centers, churches, libraries and hospitals. Workshops are facilitated by two trained leaders, one or both of whom are non-health professionals with chronic diseases themselves. Sedgwick County currently has nine leaders and five master trainers certified through the Stanford University model.
In addition to implementing the chronic disease self-management program, the Health Department will also seek to develop and sponsor a coalition around the issue of chronic disease. The coalition will include individuals and/or organizations with a common interest in assisting people with chronic illness.
