CONSIDERATION OF A GRANT APPLICATION IN THE AMOUNT OF $2,237,773 TO THE KANSAS DEPARTMENT OF HEALTH AND ENVIRONMENT (KDHE) FOR SERVICES PROVIDED AT THE SEDGWICK COUNTY HEALTH DEPARTMENT.

RECOMMENDED ACTION: Approve the electronic submission of the grant application and authorize the Chairman to sign all necessary documents; including the grant award agreement containing substantially the same terms and conditions as the application; and approve the establishment of budget authority as provided in the financial considerations section of this request at the time the grant documents are executed.

Grant Summary and Timeline: KDHE provides Aid to Local (ATL) Grant Funding opportunities to support specific or targeted health service needs on an annual basis. The ATL Grant application opportunity covers many, but not all, of the grants awarded from KDHE to the Health Department. Grants from KDHE and other sources account for approximately 62% of the total Health Department budget. This grant represents 15% of the Health Department budget and 24% of all grants received. The Health Department was awarded $2,071,147 for the previous SFY 2014; we are requesting an increase of 8% or $166,266 above the SFY 2014 Award. Without this funding, many programs would be drastically reduced.
The Health Department is requesting ATL funding in the amount of $2,237,773*; combined with the required match of $398,080 for a total of $2,635,853 for the SFY 2015. (See Attachment 1 - Chart 1 provides an overview of funding by category requested for SFY 2015, as well as a list of the funding awards received for the SFY 2014.)
Three grant categories require match funding:
a) Chronic Disease Risk Reduction (#2) has a 25% match requirement;
b) Family Planning (#5) and
c) Maternal and Child Health (#17) both have a 40% match requirement.
Additionally, the State Formula (#1) block grant includes a maintenance of effort stipulation that specifies the amount of state formula will be reduced proportionately, if the local funding for the Health Department is decreased from FY 2014 to FY 2015.
The Public Health Emergency Preparedness (PHEP - #3) Grant has been incorporated into the ATL bundle for SFY 2015. KDHE has communicated the PHEP grant application documents will not be available before the ATL grant application deadline of March 14, 2014. KDHE has requested agencies communicate their intent to apply for PHEP funding at the time they electronically submit their ATL grant application. The PHEP grant portion of the ATL is $326,597, based on SYF 2014 funding; making the total of the SFY 2015 ATL Grant Application $2,237,773 for the Health Department.

Budget Authority Adjustment: Grant and matching property tax funding totals $2,635,853.00. No additional funding from the General Fund is required or requested for the 2014 County fiscal year. Accept grant funding and adjust budget authority as necessary based on additional grant funding.
Funds will be managed through the following cost centers: 38001-274 and 38003-274 for State Formula (#1); 38005-110 and 38023-274 for Chronic Disease Risk Reduction (#2); 38069-274 for Public Health Emergency Preparedness; 38010-274 for Family Planning (#5); 38011-274 Sexually Transmitted Disease (#16); 38009-274, 38017-274 and 38122-274 for Maternal and Child Health (#17); 38015-274 for Opt Out HIV Testing (#20); 38114-274 for Personal Responsibility Education Program (#24); 38007-274 for Immunization Action Plan (#34); and 38060-274 for WIC IAP (#35).
Personnel considerations: No additional staff will be requested with this grant application. Funding from the Aid to Local grant is used to support 28 health department employees.
Expected measurable outcomes: Please see Attachment 2 for all outcomes.
Alternatives: Without these grants, local funding would have to be increased or the Health Department would be forced to reduce the level of services currently offered in many vital programs.

Legal considerations: The Aid to Local Grant administered by KDHE is statutorily authorized by K.S.A. 65-241, et. seq. Per the County's Grant Policies and Procedures, due to the dollar amount of this grant application, it requires approval of the Board of County Commissioners. Approval is by simple majority vote.
